1. Sample letter: Changing your insurance model

You are staying with your health insurer but would like a different model for your basic insurance, for example switching from the standard model to the family doctor (GP), HMO or Telmed model, or back again.

Good to know
  • The change of model usually takes effect on 1 January.
  • Switching to a cheaper model with restricted choice of doctor: possible until 31 December with most insurers, and with some also during the year.
  • Switching back to the standard model: your letter must reach the insurer by 30 November at the latest.
  • The exact deadlines are set out in your insurer's terms and conditions. Before switching, it is worth checking the premium calculator.
Click the parts highlighted in yellow directly in the letter and replace them.

The deductible and all other details of my insurance remain unchanged.

Please confirm the change in writing and send me the new policy.

Yours faithfully,

2. Sample letter: Raising or lowering your deductible

A higher deductible (franchise) lowers your premium; with a lower one you pay less out of your own pocket when you see a doctor. The change always applies from 1 January.

Good to know
  • Higher deductible: your letter must reach the insurer by 31 December at the latest.
  • Lower deductible: your letter must reach the insurer by 30 November at the latest.
  • Deductibles for adults: CHF 300 (standard deductible), 500, 1000, 1500, 2000 or 2500. For children: CHF 0, 100, 200, 300, 400, 500 or 600. Not every insurer offers all levels.
  • The premium calculator shows which deductible pays off for you.

3. Sample letter: Notifying a change of address

After a move, your health insurer needs your new address for your policy, premium invoice and insurance card. Enter your new address as the sender above.

Good to know
  • Report the move straight away, ideally before the moving date.
  • Your premium depends on where you live (canton and premium region). It may go up or down when you move. Compare premiums at your new place of residence in the premium calculator.
  • With the family doctor or HMO model you may have to choose a new practice nearby.
  • If you move abroad, the insurer will ask for the deregistration confirmation from your municipality.
Click the parts highlighted in yellow directly in the letter and replace them.

Yours faithfully,

4. Sample letter: Sending in reimbursement receipts

If you have paid a bill from a doctor, therapist or pharmacy yourself, send the reimbursement receipt to your insurer. It will refund its share after deducting the deductible and your co-payment (Selbstbehalt).

Good to know
  • Send in the reimbursement receipt (usually the original) and keep a copy. Many insurers also accept receipts as a photo in their app.
  • You can collect receipts, but submit them as soon as possible. In basic insurance, the claim lapses after five years.
  • Bills for benefits from supplementary insurance (e.g. glasses, fitness, complementary medicine) go in the same envelope.
  • After an accident, the insurer also needs an accident report.
Click the parts highlighted in yellow directly in the letter and replace them.

Enclosures:
– [number] reimbursement receipt(s)

Yours faithfully,

5. Sample letter: Suspending accident cover when starting a new job

If you work at least eight hours a week for the same employer, you are compulsorily insured against accidents through your employer (UVG/LAA). You then no longer need accident cover in your basic insurance and save on your premium.

Good to know
  • Enclose confirmation from your employer or ask whether your insurer has its own form.
  • The premium is usually reduced from the month after your notification is received; it is not reduced retroactively. So report your new job right at the start.
  • If you work less than eight hours a week, accident cover remains included in your basic insurance.
  • When the job ends, reinstate accident cover: sample letter for reinstatement.
Click the parts highlighted in yellow directly in the letter and replace them.

Suspension of accident cover in basic insurance

Dear Sir or Madam,

Since [DD.MM.YYYY] I have been working at least eight hours a week for [employer's name and location]. I am therefore insured against occupational and non-occupational accidents under the Federal Accident Insurance Act (UVG).

I therefore ask you to suspend the accident cover in my basic insurance with effect from [DD.MM.YYYY] and to adjust the premium accordingly.

I enclose confirmation from my employer. As soon as my employment ends or my working hours fall below eight hours a week, I will contact you again.

Please confirm the change in writing.

Enclosure:
– Confirmation from my employer

Yours faithfully,

6. Sample letter: Reinstating accident cover

If your job ends, your working hours fall below eight hours a week, you retire or you become self-employed, your basic insurance needs accident cover again.

Good to know
  • Your employer's non-occupational accident insurance continues for 31 days after the last working day on which you were paid (extended cover). Accident cover with your health insurer should start immediately afterwards.
  • For longer breaks (e.g. a trip around the world, unpaid leave), you can extend your UVG cover by up to six months with an extended cover agreement (Abredeversicherung) from your employer's accident insurer.
  • Report the end of your job in good time. You owe the premium for accident cover from the end of the extended cover, even if you report it later.
Click the parts highlighted in yellow directly in the letter and replace them.

Reinstatement of accident cover in basic insurance

Dear Sir or Madam,

My employment with [employer's name and location] ends on [DD.MM.YYYY]. [Or: From DD.MM.YYYY my working hours will fall below eight hours a week.] As a result, my accident insurance under the UVG also ends.

I ask you to reinstate accident cover in my basic insurance from [DD.MM.YYYY, 31 days after the last working day].

Please confirm the change in writing and send me the updated policy.

Yours faithfully,

7. Sample letter: Suspension during military or civilian service

If your military or civilian service lasts more than 60 consecutive days (e.g. recruit school), you are insured by the Military Insurance during that time. Your entire basic insurance is then suspended and you pay no premium.

Good to know
  • The suspension only applies to more than 60 consecutive days of service. For shorter service (e.g. refresher courses), basic insurance continues as normal.
  • Report your service as soon as you receive your call-up and enclose a copy.
  • Supplementary insurance generally continues. Some insurers offer a reduced premium for the period of service, so it is worth asking.
  • After your service, basic insurance resumes. Report any changes to the end of your service (e.g. early termination or extension) immediately.
Click the parts highlighted in yellow directly in the letter and replace them.

Suspension of basic insurance during military or civilian service

Dear Sir or Madam,

From [DD.MM.YYYY] to [DD.MM.YYYY] I am doing [recruit school / military service / civilian service]. The service lasts more than 60 consecutive days. During this time I am insured by the Military Insurance.

I therefore ask you to suspend my compulsory health insurance for the duration of my service and not to charge any premiums for this period.

My supplementary insurance should [continue unchanged / continue at a reduced premium during my service, if possible].

I enclose a copy of my call-up. Please confirm the suspension in writing.

Enclosure:
– Copy of call-up / marching orders

Yours faithfully,

8. Sample letter: Notifying the birth of your child

If you registered your child before the birth, send your insurer the final details after the birth. Only then will it issue the policy and insurance card for your child.

Good to know
  • Enclose a copy of the birth certificate from the civil registry office or, for the time being, the hospital's birth confirmation.
  • Thanks to the prenatal registration, your child is already insured. Even so, send the details within the first few weeks after the birth so that bills from the paediatrician and hospital are processed correctly straight away.
  • The two highlighted paragraphs on supplementary insurance and bank details are optional. Delete them if they do not apply.
  • No prenatal insurance yet? Basic insurance applies retroactively from birth if you register your child no later than three months after the birth. Find out more in our prenatal insurance guide.
